In-Network
An in-network provider has a contract with your plan to deliver care at negotiated rates. You pay the plan's lowest cost-sharing when you stay in-network.
Network
A network is the group of doctors, hospitals, and other providers that have contracted with a Medicare Advantage or Part D plan to deliver care and drugs at agreed rates.
MOOP (Maximum Out-of-Pocket)
MOOP stands for Maximum Out-of-Pocket. It's the yearly cap on what you pay for covered in-network medical services in a Medicare Advantage plan. Once you hit it, the plan pays 100% of covered Part A and Part B care for the rest of the calendar year.
PPO
A Medicare Advantage PPO lets you see providers in or out of the network, usually without a referral. Out-of-network care costs more and has its own MOOP.
HMO
A Medicare Advantage HMO is a plan that uses a specific network of doctors and hospitals. You choose a primary care provider (PCP) and usually need a referral to see specialists.
